Now in its second month of availability, the survey is an important tool to identify programs and services needed in Jewell County. Paper copies are available at the Formoso, Jewell and Mankato city offices, and the Burr Oak and Esbon Guaranty State Banks. You may also take the survey online by visiting https://redcap.kumc.edu/surveys/?s=EH7HTTEC7F. For those who complete a paper copy, you will receive a gift certificate to a local business of your choice. For those who complete the online version, you will receive a gift certificate to the Ute Theatre within a week of completion. Please remember only one person per household may take the survey.
