Jewell County 365 will be visiting various Jewell County businesses Nov.

15 through 19 to give residents an opportunity to take the Community Health Needs Assessment. If you or someone in your household has not yet taken the survey, you can stop by and take it at their booth and win a gift card to a Jewell County business of your choice. Paper copies are also available at the Formoso, Jewell and Mankato city offices, the Burr Oak and Esbon Guaranty State Banks and the Randall Post Office.

You may also take the survey online by visiting https://redcap.kumc.edu/surveys/?s=EH7HTTEC7F. For those who complete the online version, you will receive a gift certificate to the Ute Theatre within a week of completion. Please remember that only one person per household may take the survey. The booth schedule is as follows:

Monday, Nov. 15, in Burr Oak at Grate Expectations: 11:30 a.m. to 12:30 p.m.

Tuesday, Nov. 16 in Jewell: 11 a.m. to 1 p.m.

Wednesday, Nov. 17 in Mankato at Sweden Creme: 11a.m. to 12:30 p.m.;

and Kiers:12:30 p.m. to 2 p.m.

Thursday, Nov. 18 in Mankato at Bob’s: 11:30 a.m. to 1:30 p.m.

Friday, Nov. 19 at Possibilities in Mankato: 9 to 10 a.m .; and Mankato Sale Barn: 11 a.m. to 1 p.m.
